Parents and guardians are encouraged to fill out an income survey using our fast and easy online form found at www.familyincomesurvey.com. Unlike our My MealTime service for online applications and payments, account creation is not required. A parent can submit an income survey within minutes, from any device that can access the internet. Once complete, that data is transmitted immediately to your MealTime software for review and reporting. This site is available in both English and Spanish.


We’ve made the requirements for entry as minimal as possible. Parents need to know their student’s school district name and some limited information about their student, but they do not need to know the student ID, a common barrier to successful survey participation. MealTime will attempt to automatically match the student with as much information as is provided.


If your school or school district is taking paper surveys, and you want to record them in MealTime, you can use the same quick and simple interface.



Submitting a Survey

1. Navigate to www.familyincomesurvey.com in your web browser.


2. Select the State, the School District or Charter and click Next:
Note! Click the En Espanol link in the top right of the page to change the survey to Spanish.


3. Enter the student's information in the required fields and click Next:
Note! If you have more than one student, click +Add another Student to add additional student information.


4. Enter your contact information so the school can reach you and click Next:


5. Enter the Total Annual Income for all members of the household along with the Total Number of Household Members and click Finish:


6. When the survey is complete, you'll receive a confirmation message. You can either start a new survey or close your browser tab to exit:
